Phoenix April Update

April 9, 2021

Dear Phoenix Center’s Residents, Families, Staff, and Visitors:

Hope everyone had a great week!  As of today, April 9, 2021 we have 0 active (+) resident case and we have 1 active (+) employee case, self-isolating at home. However, we do have 1 resident that is considered a vaccine-breakthrough confirmed case (patient was fully vaccinated & was (+) a little over 90 days), with proper care plan in place. We continue to work with local DOH, CDS and our infection control doctor for guidance and to maintain our protocols that were put into place. We continue to test all our residents and staff weekly. We continue to work diligently to adhere to all our infection control practices.

VISITATION GUIDANCE IS IN!   Please see attached Indoor Visitation guidelines that was created following the Executive Order. Our facility-wide testing from last week came back with all negative results! We can resume indoor visitation with some restrictions. We can always offer end of life, compassionate care, & essential caregiver visitations when requested and appointments are made in advance. As the weather is getting warmer, we are offering outdoor visitations as well! Please reach out to your social worker to get more information.  Also, we continue to offer video calls with your loved ones as well. If you have any questions on the indoor visitation guidelines, please do not hesitate to contact me or your social worker.

VACCINE UPDATE: We just completed our vaccine clinic on April 7th. The next one scheduled is on May 13th from 1 PM to 3 PM and will be offered to all our UNVACCINATED or not FULLY VACCINATED residents and staff. We strongly encourage all family members to have all residents vaccinated as it will allow more frequent visitation and not be limited to only twice a week. Pharmscript will be providing the Pfizer vaccine. Unit managers should have reached out to you by now to get consent.

We thank everyone for your continued support and patience that was provided during this past challenging year and thank you for continuing to support us! Our staff will continue to work hard to ensure we are keeping your loved ones safe and themselves from COVID-19. Hoping for a success of recovery this year!

Stay safe and healthy!

We at Phoenix Center continue to look forward to brighter days ahead!
