Phoenix January Update

Dear Phoenix Center’s Residents, Families, Staff, and Visitors:

As of today, we have 2 active (+) employee cases, quarantined at home and 1 resident case with proper care in place. Many of our staff have recovered and returned to work and many residents recovered & doing well! The facility will continue to monitor all staff and residents for Covid-19 symptoms and will take any appropriate steps necessary if deemed to be symptomatic. The facility continues to follow all CDC and DOH recommendations and continues to report specific numbers to the DOH on a daily basis. The facility will continue to test staff and residents weekly.

** THE FACILITY WILL CONTINUE TO HAVE LIMITED VISITATION TO REDUCE THE RISK OF COVID-19 TRANSMISSION FOR BOTH GERIATRICS AND PEDIATRICS. ALL VISITS MUST BE BY APPOINTMENT AND WILL BE HELD IN THE DESIGNATED VISITATION ROOM. PLEASE SCHEDULE YOUR VISITATION APPOINTMENT WITH YOUR SOCIAL WORKER OR FRONT DESK. **

Due to the increase in cases for COVID-19 and the Omicron variant the facility is taking extra precautions and HIGHLY recommend to please AVOID visitation. We recommend two visitors during visitation, and we kindly request NO children to come for visitation at this time.  Rapid test must be completed upon arrival and full PPE must be worn at all times**

Failure to follow the policy may result in NO future visitation

We are also implementing to have limited visitors waiting on the lobby, you may be told to wait in your car until the results are in. Designated staff will call you when ready.

VACCINE UPDATE: Our next scheduled Vaccine Clinic will be next month. We are now able to vaccinate children from age 5 to 11 with Pfizer Vaccine. The Social workers will be reaching out to families to get consent. See attached for information. We strongly encourage all family members to have all eligible residents vaccinated as it will allow more frequent visitation, socializing events such as more opportunities to be able to go inside the day room. Unit managers will be reaching out to you to receive consent.

OTHER UPDATE: Dayroom activities are currently suspended however all residents are receiving 1:1 visit.

RENOVATION: We are on our way to finishing up the second-floor renovation. We have started our first-floor pediatric renovation! Our goal is to ensure we are keeping our residents safe during construction. We have a safety plan that is available if anyone wishes to review it or if anyone has any questions, please be free to contact Administration.
